Severe Mandibular Jawbone Resorption

Avoiding Mandibular Nerve Damage

Our patient, a 70 year old male, had worn a lower denture partial for years. Because of the wear and tear on soft and hard tissues in the lower jaw (not unusual for long term denture wearers), the patient sustained considerable bone resorption. His remaining lower teeth were severely deteriorated.

Pain became unbearable as the denture created increasing pressure on the mandibular nerve. The patient eventually was forced to stop using a lower denture. Soon after, because of eating difficulty, the patient began to lose considerable weight.

Rehabilitative Analysis

When the patient first presented in our office, Dr. Marinic was able to determine that the patient could be a candidate for a specialized implant system that would take the best advantage of his unique jawbone structure. Some areas of the mandible were healthy and had sufficient bone material that could support a custom implant solution.

Dr. Marinic surgically located the delicate mandibular nerve and was able to develop specific treatment methods that eliminated additional risk for damaging this critical nerve tissue.

Treatment Plan: Teeth In A Day

Dr. Marinic extracted the remaining teeth in the patient's lower jaw. Implants were placed in healthy areas of the jaw. Since a specialized extraction procedure was used that did not further compromise the patient's bone structures, a total of four (4) immediate load implants were placed, enabling the fitting of an implant secured denture on the same day.

After a six month healing period, the patient returned to have a specialized impression. A verification jig is fabricated and tried in the mouth to check that the model accurately reflects the position of the implants in the mouth.

Precision is critical at this step and has to be within fifty microns tolerance. An ideal denture setup is fabricated to accommodate the new implants.

The next step involves the fabrication of a pattern. Next the pattern is sent to Sweden to be scanned and milled on a CNC router out of a hocky puck size piece of commercially pure titanium.

The final product generated from this titanium milling process is called a Procera Implant Bridge (PIB). The PIB is returned to our laboratory. There it is then polished with a special system developed in Germany.

Next the PIB framework is ultrasonically cleaned, sterilized and tried in the patient's mouth with a single screw to verify a passive fit. It is absolutely imperative that the PIB fit the implants passively.

If there is any tension on the implants it will result in tension on the implants which can result in pressure necrosis and implant loss. The tolerance is less than 100 microns.

Function and Esthetics

Then the teeth are added to the framework and a tried in the patients mouth to verify phonetics, esthetics, and function.

Failure to follow through with attention to detail can result in having a poor fitting restoration that is not only uncomfortable but can cause speech difficulties.

Edentulous patients will typically have very little cusp angulation on their back teeth. Since these are screwed in, the cusp angle can approach that of a normal tooth. This allows patients to "chew" their food rather than "crushing" their food.

The final laboratoy step involves processing the teeth onto the PIB framework in a color of acrylic that matches the gingival shade of the patient.

Then the PIB was placed in the patient's mouth and the abutment screw torqued to the exact manufacturer's specification.

An upper PIB was also created for the patient which essentially is a duplication of this Teeth In an Hour process.

After the upper and lower PIB's where placed, the patient looked at his new smile which now provided normalized support for the cheeks and lip structures and said "maybe now I can get a kiss!"

The patient was thrilled with his new teeth, his appearance and his renewed ability to eat just about anything he wanted.
